Membership Renewals


Dear CHAPA Members,
 
Renewal Season 2021/22 is here. Due to Covid19, we are requesting our renewal process to be completed online as much as possible.
 
Three documents have been attached. Please read & complete all;
  1. Membership Renewal Checklist
  2. Membership Renewal Form
  3. Declaration
  4. Complete payment by Cheque or online via Visa Or Mastercard through your profile portal. (e-transfers are not available at this time)
 
Once you have completed every requirement (updates, payment, uploading documents), Please email me a completed copy of your Membership Renewal Form & Declaration. By doing so, this will inform me you are complete and I can start reviewing your renewal paperwork.
 
Friendly Reminder: Membership Renewal is Due January 31, 2021 at 11:59 pm. Incomplete & Late Renewals will result in an automatic $300.00 late fee.

We are not requesting insurance verification for renewals as CHAPA will cover professional Liability insurance for its members for April 1st 2021.  This link is available for those who need it prior.
